The Juanfe Foundation is a Colombian non-profit organization that works to break the cycles of poverty, vulnerability, inequality, and exclusion of teenage mothers through a social 360-intervention model that offers support on health, education, and connection to the labour market. The program's success lies in the team's detailed evaluation of each teen mom's psychosocial situation and the definition of the personalized intervention and support plans for each.
The lack of tools or skills to process the data volume of their intervention model hinders the monitoring and evaluation of each case and the program in general.

Together with Deloitte and Salesforce, OT team from Colombia have created a platform that efficiently manages thousands of cases, performs accurate and timely follow-ups, makes decisions based on real data, and multiplies its impact without losing its human essence.
This small contribution will help the Juanfe Foundation continue to impact more than 2,000 young mothers in five cities with its comprehensive “Modelo 360” program.
